How the Service Works

    • Refer yourself or someone else for an assessment by completing this form
    • Once you have made the referral, your information will be processed and you will be assigned an assessment officer.
    • You will be contacted and offered an appointment. Due to demand, this appointment might be several weeks after your referral but we will see you as quickly as we can.
    • Your assessment officer will discuss your needs and the options available at your assessment.
    • Equipment ordered following an assessment will be supplied and installed by a company called Medequip. They usually deliver within ten days, but it may take longer if the item(s) are out of stock.  If you need to speak to them please contact 01322 917909 or email kent@medequip-uk.com
    Please be aware that KCC has placed strict limitations on the equipment which can be provided following an assessment, such as the exclusion of many items previously provided. Your assessment officer can discuss this in detail with you at your assessment.
